I think a nice visual indicator of how many days your character has survived would be for a beard to slowly grow culminating in a full Jesus beard so if you saw anyone with such a beard they would instantly command respect and you would know not to mess with them. Might also be something of a goal - who can attain the holy jesus beard.

if they made it grow by logging a certain amount of in-game time, that would be a little harder to do. a huge waste of time either way though.
