If you haven't been drinking that's the reason you're feeling dizzy.

Water pumps are bugged in the current release I believe. I think they're fixed in the next patch though.

 

If the graphics are going fuzzy from time to time it's probably an effect of your character's physical condition. If they're causing you problems (I find they give me migraines) you can turn down/off post-processing in the graphics options.

It's a bug where you can't drink directly from the pump sometimes. The easiest way is to find a bottle or canteen and fill it instead.

When should I be extremely worried about dying of thirst? Would my screen be black & white? I've been playing almost 2 hours with only food.

Do you guys think I should be able to make it to the swamp if I'm feeling dizzy, I need a drink, my head is throbbing sort of things?

I've learned you only REALLY need to eat or drink when it says "extremely" or is in red text. Otherwise it seems like it just says that if your hunger/thirst aren't at 100%.
