UK 306 meets all but one of your requirements. It is hosted in UK, but it's on a dedicated machine, so the lag is not bad. It usually has around 5-20 players during the day up until about 11 PM (UTC/GMT -5 hours).

I created a new server today, and I really believe that it stands out from the rest of the servers, I have a custom loadout, it's near bare-bones though. I currently have 2 admins, but I'm open to more. I have a donation system (with perks) and it's in a timezone I haven't seen in a while.
