Do you lose the Ghillie suit if you equip a different set of clothing?

It returns to your inventory, in the little package saying "Ghillie suit".

So no.

No, it will return into your inventory in a package which you can re-equip anytime.

No it swaps the current clothing you wear back towards in your inventory same clothing package thingy.

No I think they get swapped in your inventory. So you put the normal cloths on and the ghillie suit gets put in its place in the inventory. I have no personal experience with it however so don't take my word but just what I have observed from videos.
