Yes, they are possible. From massive shock. You will see messages about your status changing in various ways letting you know something is wrong. A Defib will save them prior to seeing "you are dead".

You can also give a player a heart attack and Ive been looking for my 1st victim for a while now. You defib them while they are healthy.

Heart attacks are very possible, indeed. I think they're caused by too much shock (caused by damage taken). I was healthy and in NWAF a few weeks ago and after I was hit by a zombie I got messages like "my pulse feels irregular" and "my chest hurts". Shocked as I was (you may laugh after reading that epic pun), I sat as still as I could and made absolutely sure I wouldn't receive any more hits. As soon as you see those messages just don't take any more damage (shock), otherwise you'll get the message "I feel like passing out", after which you fall unconscious and only a defrib can save you. Luckily I didn't get the latter. I'm not sure about the black and white screen, tho
EDIT: I suppose the black/white screen makes some sense, considering your heart stops and your brain doesn't receive enough blood

Heart attacks will not cause a black and white screen, though they do exist and can cause death should they not be treated properly.

The Black and White screen is from being shot. Not every bullet causes bleeding, though they do have a higher chance. That chance can be lowered depending on the condition and type of your clothing, with military clothing and vests (namely press and ballistic) helping the most to prevent bleeding.

What probably happened was your friend took a few too many hits and lost a bit of blood and health. Loss of health means your screen will go blurry. A loss of blood means your color saturation declines. This is normal after taking a few body/leg/arm shots and very common after taking a bullet to the head and surviving. Your friend most likely died from a heart attack but he probably had terrible blood loss after the fight causing that black and white screen.

Just some elaboration on where it comes from: every hit you take doesn't only diminish your health/blood, but also adds shock. If you take many hits, the shock reaches a certain amount, at which you'll start seeing those messages. If you still take hits after those messages, you will see the message "I feel like I'm going to pass out" and you will then pass out with the defrib as your only saviour. Since the player is onconscious I'm not sure if an epi pen would also work, but never got to try it. I don't know how shock is treated in-game, but my best guess would be to (obviously) not gain any more shock and (not sure about this) don't move. I immediately reacted to the messages by sitting still and it seemed to have worked. This means that fatigue may have play in whether you pass out or not (I was also fatigued when I took the hit from the zed which caused the messages).
