Ruined means worthless and pristine saves you, I have not tested everything in between.A bullet that is stopped by a helmet has the chance to knock you out (make you unconcious). I am not sure if different bullets have a different effect here. Anyway, after one hit the helmet is most likely ruined and worthless.
