Sometimes items fall through the ground.  It was a sad day the first time I lost an ammo can this way.  Thankfully, I realized that 556 ammo is far from sparse and began to just throw it away to keep it out of the hands of others.

Thing is I can see it but can't grab it. I see the whole gun but ....

 

I waited around 5 minutes just trying to pick it up and then gave up cause I just had a gun fight recently and was in a high traffic area.

70h ingame, shit tons of loot dropped to ground and i have never lost anything due to dropping through ground. Sometimes it just takes up to 30-45sec to loot to appear.

I had one double magazine on m4 and one with 40 bullets in inventory, while trying to open a door i accidently reloaded my weapon and double magazine vanishes, i waited some time (about 2-3 minutes) as i was thinking it was "item spawn time delay" bug, but no avail.
