One hitted by axe

Imagine this situation:

 

I'm completely healthy and I have just drank something and wasn't hungry at all. Then I spotted a Player with some backpack and a axe which wasn't equiped though. I followed him some time and in the right moment I attacked him with my axe from behind. I hit him at least two or three times before he started to run away. While following him I could hit him one more time and he was bleeding like hell. Suddenly he decided to stop running. I thought he gave up but he equipped his axe ran around me and hit me only one time and I lost consciousness :D So I'm wondering now how he did that. Does it make a difference which part of the body (head, legs, ...) I'm attacking with the axe or was it just luck?

I killed a guy yesterday with a pen in one hit, it was the single greatest thing ever getting to yell "THE PEN IS MIGHTIER THAN THE SWORD" then stabbing him in the face as he tried to get me to surrender, he had a mosin so I figured if I get close enough he can't shoot at me properly. I was right.
