According to the wiki, it only fires HE grenades, but I swear I've seen smoke grenades for it.

What does it fire and what is the best way to use it?

Hmm.. Ran into 3 M16's with M230 options..

Still prefer my M4 with the reddot scope though, heard it can be tricky to shoot?

A little. The grenade sight is permanently upright, and it takes me a small amount of extra time to see where I'm aiming.

Hmm interesting.. I still prefer a scope over an iron sight though :\

The leaf sight is also completely non-functional, so don't try to use it to aim.

Yeah, I love the style of the M16 + M203 but the leaf sight blocks a lot of your iron sight when you are trying to shoot straight. Plus, I have never come across an HE grenade in the time that I have had the grenade launcher, I have only found flares and smoke.
