Are bans by IP or by GUID?

When I ban someone from my server as Admin, is it banning them by their GUID, the IP, or both?

I banned someone and I am pretty sure the same person was back on in 10 minutes time...

Ban by GUID because IP could end up banning an innocent player who gets assigned that IP at some point, if they come back with a different GUID then they're using stolen keys.

If they do keep coming back with different GUID's then you could temporarily ban the IP as well to stop them but remember someone else could get that IP at a later date.
