Entry in setpos.log - is this a ban?

I still don't know what exactly I am looking for in this log, but I have an entry here:

26.09.2012 14:25:06: PLAYER_NAME (PLAYER_IP) PLAYER_GUID - 69:7 [12258,9506,20]

Does this mean somebody is teleporting or not?

I wouldnt ban anyone on a single file entry. What you want to look for is a pattern of small time usage over a distance.

as I understand the log if it is a flood of say 6 or more entries, that's probably bad news, a single entry is supposedly chem lights and flares being picked up and thrown

Flares and chems started and thrown by the same player do not trigger setpos, those by other players supposedly do. I have personally tested my own flares and chems and they dont show up, not sure about others.
