Fix players dying on spawn?

This is a well-known glitch. After you connect and load in, your player dies at the instant you spawn. Then you spawn in the debug forest, then you abort and respawn on the coast with zero gear.

Some servers do this and some don't, it seems. How can I make my server one that doesn't?

You've already answered your own question, it's a glitch, not much you can do about it. However, your gear is on your dead body still. You just have to go back to where you logged off at.
