Yes.

Hive hicupped, stalled the net-stack thread and queued events.

A client disconnected, or a zombie was killed, or a gun was picked up during that time and the queued events were then processed.

But the state of the object had changed, or the object was removed - therefore it's not found.

that's my best guess ;)
