Is it possible to head shot (one shot kill) a player? or I have to shoot him until his HP dropped to 0? Does head shot only apply to zombies?

Yes you can headshot a player. And, from my experience, it's 100% lethal.

Don't always aim for the head though, firing at the center of mass (torso) is much easier and is still fatal.

That's more likely due to lag, if you or the server is lagging it might take a few seconds for them to go down after they get shot.

Yes it is possible, it is hard to do though depending on your experience in firing weapons in Arma 2 and also what weapon you are using.
