Is this a bug or did i do something wrong?

i died by another player after logging in, then i aborted, disconnected, tried respawning, took to long at loading screen so i closed combined operations, then i got back on an hour later, logged into a random server, it was loading never asked me what gender i wanted, and it said i was in prigordky something like that and i just fell over unconcious and started losing blood.

This has never happened to me before so i don't know what i did and if there is a way to prevent this in the future.

p.s. i was already dead so i shouldve respawned
