Got teleported to somewhere (couldn't see, was at night, around 00:30 GMT-3), got shot and killed.

I saw a "X killed Y (friendly fire)" message, the killer nick was a Pacmania (or something like that), I don't know if it haves something to do with it.

He was global banned yestereday, 3 minutes later he joins the same server somehow not banned anymore. Today he killed me and a buddy (10:30 PST). Not Sure how, no bullets were fired... both died at the same time. Perhaps you can check the logs on that server.
