Really Choppy gameplay?

It was working perfectly as i sorted out my super lag issue, now when i play it will go on for 4 seconds and pause for 6, constantly, can't move, open pack, etc... But when i hit Esc and press abort, before aborting it runs so smoothly in that 'Esc' menu, just the in-game is really glitchy/choppy, every single server, tried re-installing, updating, everything, can't play:/ any solutions or fixes? Don't just give me the obvious

get a graphics card ^^ a 1gb runs it good.
