Tips on Avoiding Item Loss When Rejoining?

Ive had several incidents where my stuff in the backpack are transformed (alchemy/hacks?) or lost on rejoining. The most recent one I had my MP5SD transform into AK74 and my medical supplies and food changed into M1014 Slugs (I have M14 Aim as equipped primary) and now im left in the wilderness with no food and a brownish food icon :(

So, any ProTips on how to lower the chance of such glitches/bugs? Or can someone shed some light on how this hive thing works (like is my stuff sync'ed every 10 mins or something?)
