I got this when I died, and can't figure out how to do this, short of crashing the game. I can't even hit escape to bring up a menu. Am I missing something?

Its known, it happens every time. You have to press ALT-F4 to quit the game.

My problem with this is I don't know if I'm suppose to Abort and go back to the very beginning at the menu or what?

go back to player lobby and select a slot :)
