I guess what he means is that when you start up a game for the first time, the volume is always too high.

I always lower volume when I start up a game for the first time.

I just turned the 'Music' volume slider all the way down. It was killing the mood when it sounded like someone was watching 'Lord of the Rings' right behind me everytime I played. Immersion breaking. I prefer to have nothing but the sound effects in the game. Makes it far more immersive.
