Random death bug? Community server

Hi all, so we are a group playing on a private server / hive.

twice now the same person have encountered just suddenly dieing when entering an area in a vehicle. And in the .adm files it just states the person died, 0 bleed sources, hp full, water and food fine.

the line before does not state the player being hit or anything.

first car was a gunther, the second a hmmv
