Combination lock falled off by itself

Hello,

I play on a vanilla private server and we experienced a weird thing yesterday.

I logged back into our base and one combination lock (4 digits) was off and it seems it must have fallen by itself. Is that possible and why does that happen? A few more notes:

- it couldnt have been breached because nothing was missing and all other 3 gates were fine. This one was the last one so in order to get there you have to get through the first 3 at first.

- it did not reset because it had the same code still set

- we are sure we did not forget to put it on

- a few days ago we raided a base when we noticed that there was a code lock lying right by the gate. And it was a code for the rest of the gates as well, haha. The fools set the same code for all 3 gates. But it looks like the same case, that it just fell off.
