Adding Custom Loading screen

I am having a bit of a problem trying to upload a custom loading screen to my server.

I have tried to follow the steps of this:

However still no luck.... Maybe i'm just having a bald moment and its not all registering in my head.

 

If there is someone who has added a custom loading screen to their server before if you could maybe give me a walk through talk through guide then I would REALLY appreciate it as I've spent hours trying.
