Random death?

Is this a known issue at the moment?

 

Was running along some road and I just got the black screen and a message saying I'm dead. Was perfectly healthy, and 100% sure I wasn't killed by another player.

 

Have the game since 2014 but play it pretty inconsistently... So made a forum account just to ask if anyone else has encountered this?

 

Edit: Sorry for pestering on the forums. Have submitted a feedback report, couldn't find the star earlier to get to the page but you had to be logged in there to see it. Feel free to close the thread or whatever.

I saw other tickets from people with the same issue and someone replying it sounds like they were shot in the head, but I'm sure I wasn't killed. It'd only take someone a couple of minutes to confirm from the server logs if I was shot, and if that's the case I'll apologise for wasting time.
