Losing health even when completely full/haven’t been hit?

I’ll be losing health like 5 mins into a new character even if I eat and haven’t been hit by anything. Is this a glitch or am I doing something wrong

you may not be able to see if you are dehydrated, because I was having the same problem then I found out that I could not see what my water level was very low.
