Run the game in singleplayer (Sometimes, servers that have been on for a long time get laggy due to scripts.) and tell me if the lag is still persistent. What is your GPU? And, make sure Video Memory is set to "VeryHigh"

Well, I know that your GPU is on the lower spectrum. Maybe it's time for an upgrade?
