In the same apt I fell through the last set of stairs on the first floor and wound up in a room that is normally inaccessible.

 

My fix - laying flat and crawling through an impassible glass wall I can't escape through if crouched or standing.

 

  The second issue was descending the ladder from the roof.  At the point you transition out of the elevator shaft I could not get out.  I tried stepping over, toggling up down, crawling didn't work, etc. 

 

My fix - I turned around and backed out.  It worked, but then the server crashed or whatever.
