Hacks or Bug?

I spawned in Elektro on the hospital roof and waited there to scout it out before i made my move to go loot. A man spawns in front of me with no pants, a mosin, and a stupid down coat. I shot him once in the back with my mosin and he fell down. I shot him again in the back and then once more in the head to make sure he was dead. 

 

I went move literally 4 steps forward to loot his body and he stands up, turns around and shoots me in the chest. I shot him once more in the chest and he shot me again in the chest and i died. I hear him bandage and loot my body.  

 

 

What the hell happened here????

This doesn't explain how i shot him 4 times with almost no armor on and he shot me twice with full gear on and i died.

 

I was full health as well.
