its a known desync issue man thats been haunting quite a few of us since last patch.  I twitter hit the devs, hopefully we'll hear something soon.

If the hot bar isn't working drag it from the inventory screen into your hands.

That doesnt work either.  Its weird it shows the item in our hands to other players but our client doesn't actually put the item in our hands.  Nor can you pick up any items.

Desync...mostly caused by low bandwidth afaik... only solution I found was to only run around in small groups and hope you don't come up against a big bandit group in the vicinity.

 

It's also affected by the number of containers in your group (medkit/ammo case/protective case)
