Killed by nothing?

So i was in Elektro, i killed a random guy. Suddenly in the fire department building i got the screen "You are dead". The funny thing is that i haven't heard a single shot and i am sure i was the only person in there. The guy i killed started to talk to me, i coulden't really understand him so i continued my journey

 

Anyhow i respawn, and suddenly it happend again, "You are Dead".

 

Now im left with one question. Was a hack used, or what the hell happend?

ive been killed many times where i didnt hear anything. I always remain in the "dead" screen to listen if someone loots me and everytime i just die randomly there is always someone looting me shortly after. So maybee the sound is bugged? or you simply die before you hear the bullet
