So what is the system behind item damage?

Earlier at Balota tents I shot a guy clear in the head and when I went to loot him I noticed EVERYTHING on his was ruined.

 

This is just one example of me seeing this happen from a single headshot but other times it doesn't.  I've shot people with multiple bullets from an M4 or pistol and only a few things are ruined, but when headshot ruins everything?

 

I just do not understand how to know what you will damage if you shoot someone or where to put your belongings in case you get shot.

Are you certain he was not equipped with damaged items before you shot him?  I dont kill players so I have never seen this happen.
