Character continuously getting wiped?

For the past 2 days, it seems every time I log in, I'm a fresh spawn. It's happened to me probably 10 times already, and I'm not sure what's causing it.

 

I always log out or get red-link disconnected while i'm at good health, no thirst or hunger issues, but then once I log back in, i'm a fresh spawn.

 

I'm also not talking about the global character wipe which happened the other day, I did experience that, but ever since that patch, I've been unable to continue where I leave off.

 

Just curious if this is a common bug going on atm, or am I somehow triggering this myself? (logging out too close to a friend perhaps?)
