How exactly do you update?

I've only downloaded and extracted day z once and now I'm confused on how to update. Do I have to download and extract everything once more for every hotfix or patch?

I use six updater, it's noted on the DayZ download page, makes it a lot easier for me to update because it automatically updates the Beta. you should try it, just go to Download on the DayZ homepage, and select Six Updater thingy. there is a guide below it how to use it.

I update manually.. it's quite easy and I don't have to rely on other tools

you only have to download the updated files which are usually mentioned in the related thread

If you're doing it manually

1. Download newest patch

2. go to your dayz folder

3. replace old files with new files that you just downloaded

I find that it is easier to download through torrent so that you can download everything together.

Also if you download it in torrent then it will come in an @dayz folder so you can also just replace the entire @dayz folder if you want to.

Edit: you may also need to download arma 2 oa beta patches to get new dayz patches to work
