Thats the hero skin, you get it if you have high humanity. You can't kill innocents, murder many bandits, and lots and lots of blood transfusions.

Looks like the hero skin. You get that from throwing haybales about, carrying puppies out of burning buildings and pushing chicks along on pianos.
