Items resetting after I leave server

Playing on the same server, which as far as I know is official. I'm hurt with a broken leg/bone or whatever. However, I got a friend of mine who is on his way to come help me since he has a morphine injector. So I jumped on the server earlier to see exactly where I am and I've lost my items I was carrying before I disconnected. Its like I'm starting with only the items you start when you start the game (bandage, pain pills, flashlight, backpack) yet I am still in the same location and hurt like I was when I left the last time.

Another friend of mine, who is new to the game, was with me. He also had his items disappear when he left with me and we joined back.

What gives?
