Tetanus is a bacterial infection which attacks the central nervous system, I don't understand the correlation between a vaccine for it and the provided benefits (it would kill you soon after infection if not preemptively controlled/killed i.e. vaccination). That said, vaccines often require special care, refrigeration, and are generally available in nations with good health care systems and low poverty rates (because they are so expensive to correctly store and monitor). You will never be given a vaccine which has exceeded a certain temperature, doing so can seriously injure or endanger you, all improperly handled vaccines are handled according to the companies emergency procedures.

Also if you think living in a dirty place hurts your immune system you are mistaken, it does the exact opposite, your immune system needs to be challenged to become stronger, think of it as a learning body system. If you were a child kept in a bubble (or a clean environment all the time) when you were exposed to an unclean environment you would become extremely ill; that's why it's good to have a child crawl on the floor and be disgusting, because it builds "immunity."
