    I have the exact same problem. I don't belive it's FADE. The problems isn't that the shots are inaccurate, fully rested + prone, single shots will all hit the same place. The problem is the iron sights are off, almost like the entire iron sight overlay was moved down slightly. Edit: I went into the OA armory and everthing was normal. Edit 2: I played on a server with crosshairs, the bullets hit exactly where they were supposed to as long as I didn't use iron sights. The iron sights on the makraov were fine, so as far as I know it's only the 249SAW and M16 with messed up iron sights for me.
