No. 1 is a bug, you have to log out and back in, seems to happen on some more servers than others. No. 3 you're better off finding a Teamspeak/Clan or a small group of friends to play with than randomers. Number 4 is a hacker, he teleported you and the rest of the server in the air and killed you all, it's fairly common-place now.

I've been killed at least three times by a hacker, each time 5 minutes before a restart, it's like the bastard doesn't even want us to be able to re-loot our stuff. -

I can imagine most diary entries being: "If you find this, you're probably the guy that killed me, fuck you!" But yeah, it'd be something interesting to do in some down-time.
