Talk to eatch other (using team speak, steam's voice call, etc.) so you know where everyone is ("I'm at ... and just going to ..."). That's how me and my friends do it. When you know where everyone in your team is, you don't hesitate to take out another player when one suddenly appear in front of you. IMO adding visual clues like friendly tags would just take away from the game. -

nonpersistent tools and possibility to fail at using them

I'd like to see tools such as matches to be nonpersistent. So when you find a box of matches it would have for example 10 matches in it. Also there should be a chance to fail at using them (especially if it's raining). Also things like knives, axes, etc. could wear down in use. Currently you find those things once (per character) and don't need to care about those anymore. This change would make those items more valuable since you would have to get new ones once in a while (meaning more trips to towns). The chance of failing could be extended to converting bullets. For example when you're converting revolver ammo into 1911 ammo there could be a 5% chance of destroying the ammo. The chance for failing would be higher when playing around with ammo for the more powerful weapons. -
It would seem so to someone without any knowledge of software development process. In many cases a regression bugs are not caused by a single change and the root cause is hard to find. Blindly reverting things instead of figuring out what broke it and why blocks actual progress on the project. When you're testing something under development, there will be regression, new bugs, etc. They get investigated and resolved after a proper fix (or a series of fixes) is found. Also quick and dirty fixes quite often introduce other bugs. Patience is a virtue also in software development. just my 0.02
