If you search the forum, there have been numerous reports of people being killed by trees and foliage. It appears to be (yet another) documented bug in the game. Just suggesting another possible explanation. -
Selecting gear in the way you just described is useful in the following situation: If you are riding in a car, heli, or other vehicle (most likely as a passenger), and you find yourself needing to eat or drink, you can access your inventory this way without getting out of the vehicle. Pressing 'G' when in a vehicle will not open your inventory, so you must access it through the map menu.

I'm pretty sure in the in-game item description for the hatchet it says that the hatchet is one-handed, so making it a secondary would make sense.

I'm pretty sure the way it works now is there are 3 potential heli spawn locations on the map, and a total of 2 helis spawn at any two of the 3 spawn points. -
