Have you tried following one of the guides in the forum? Before the update to the forum? Or did you go at it alone?

From what i understand from that post, you've been shot 3 times in Elektro or been killed or randomly died. By the sounds of it being in different servers i guess it's possible its silenced weapons but even that isn't very likely.

Lol sounds like soemthing else that could be added to the server list name. Or indeed fixed

Still could be cos server side updates need to be done by the admins and they have not got around to it yet

It can be due to bugs making the server crash, use to do it all the time when hatches and crowbars were introduced, it started ages ago not just now
