Primary weapons take up 10 slots in a pack. If you don't have the 10 spaces, you lose the gun. When you disconnect, there is a chance you will log in next time 2-3 minutes before you logged off previously. So if you picked up the gun and disconnected right after, then you could lose the gun. Wait a few minutes before disconnected if you find loot. :)

Define safe and good quality... No hackers? Good ping? Can't help you with the hackers. Look for servers with <100 ping? All servers are hosted by random people. If they have a server, they get in touch with staff of Day Z and they get it set up.

This is a bug that happens to some people. Direct Comms is still buggy. Try a different server? Is your friend fully updated too?

Process might still be running. Open Task Manager and end the process for Six Launcher.

Is it spawning in weapons and such? no. Is it harmful to the game and or experience to other players? no. Not a ban reason. If Battleye didn't pick it up as a "hack", then you should be fine.
