HD 6970 8-10 FPS (city) 15-20 (open field) what wrong?

Help me, its basically unplayable my specs are: HD 6970 6gb ram windows 7 x64 AMD Phenom 2 x4 945 processor 3GHz i also run dual monitors but play on one. Please tell me what i can do, ill explain settings if needed.
